Will you please tell me that how we can add control on forme by code.

Dim Cube As Control
Set Cube = frmMain.Controls.Add("PivotCube", "Cube1")

If you have any idea please let me know what name should i use instead of "PivotCube" int above command lne.

I do not sure about your code, but you should see how to create ActiveX controls at runtime in your development guide.
Something like

Cube = CreateOleObject('PivotCubeXLib.PivotCubeX')
